Follicular Monitoring in Bihar
Serial transvaginal ultrasound for ovulation tracking at JP Diagnostic — essential for natural cycle tracking, IUI, and IVF across Patna, Gaya & Muzaffarpur.
What is Follicular Monitoring?
A series of transvaginal ultrasound scans performed across the menstrual cycle to track follicle development in the ovaries, measure endometrial thickness, and precisely time ovulation for natural conception, IUI, or IVF cycles.
Follicle Tracking
Measures the size and number of developing follicles in both ovaries. Dominant follicle typically reaches 18-24 mm before ovulation.
Endometrial Assessment
Evaluates endometrial thickness and pattern (trilaminar vs homogeneous). Optimal thickness for implantation is 8-12 mm with trilaminar appearance.
Ovulation Timing
Identifies pre-ovulatory follicle rupture (follicle collapse, free fluid in POD) to determine the precise fertile window for timed intercourse or IUI.
When Do You Need Follicular Monitoring?
Infertility Workup
- Unexplained infertility
- Anovulatory cycles (PCOS)
- Irregular menstrual cycles
- Luteal phase defect suspicion
- Advanced maternal age (>35 years)
- Previous failed conception attempts
Ovulation Induction
- Clomiphene citrate (Clomid) cycles
- Letrozole (Femara) cycles
- Gonadotropin injections (FSH/hMG)
- Monitoring for OHSS risk
- Dose adjustment based on response
Assisted Reproduction
- IUI (Intrauterine Insemination) timing
- IVF/ICSI cycle monitoring
- Trigger shot (hCG/GnRH agonist) timing
- Egg retrieval planning
- Donor cycle synchronization
Special Cases
- Recurrent implantation failure
- Thin endometrium monitoring
- Asherman syndrome post-surgery
- Frozen embryo transfer (FET) cycle
- Natural cycle FET
The Procedure
Each visit takes 15 to 20 minutes. Typically 3-6 visits per cycle.
Baseline Scan
TVS assesses antral follicle count (AFC), ovarian volume, endometrial thickness, and rules out ovarian cysts or pathology before starting stimulation.
Mid-Follicular Scan
Measures growing follicles (target 10-14 mm). Endometrial thickness assessed. Medication dose adjusted if needed.
Pre-Ovulatory Scan
Dominant follicle reaches 18-24 mm. Endometrium should be 8-12 mm, trilaminar. Trigger injection advised. IUI or timed intercourse planned.
How to Prepare
✅ Do’s
- Schedule first scan on day 2 or 3 of menstrual cycle
- Empty bladder completely before TVS
- Bring previous follicular tracking records
- Bring medication list (ovulation induction drugs)
- Wear comfortable, easily removable clothing
- Book follow-up appointments before leaving
❌ Don’ts
- Do not miss scheduled monitoring visits
- Do not start intercourse before trigger if advised otherwise
- Do not apply vaginal medications before scan

| Gonadotropin Cycles | 4-6 visits | Day 2-3, Day 6-8, Day 10-12, Day 13-15 |
| IVF Cycle | 5-8 visits | Day 2-3, then every 1-2 days until trigger |
Duration & Report Timing
| Scan Type | Duration |
|---|---|
| Follicular Monitoring (Per Visit) | 15 – 20 minutes |
| Baseline Scan (Day 2-3) | 20 – 25 minutes |
| Pre-Trigger Assessment | 20 – 30 minutes |

Safety & Risks
Follicular monitoring is completely safe when performed by qualified professionals.
- Zero radiation exposure
- TVS is safe and minimally invasive
- No impact on fertility or conception
- Can be performed throughout multiple cycles
- TVS may cause mild discomfort
- Multiple visits may be inconvenient
- Emotional stress if follicles not growing
- Multiple large follicles with gonadotropins risk OHSS
- Severe OHSS requires hospitalization
- Monitoring is essential to prevent complications
