MRI Defecography in Bihar
Advanced dynamic pelvic floor imaging at JP Diagnostic. Real-time MRI evaluation of defecation mechanics, rectal prolapse, and pelvic floor dysfunction — the gold standard for complex constipation and incontinence workup across Patna, Gaya & Muzaffarpur.
What is MRI Defecography?
A specialized dynamic MRI study that evaluates the structure and function of the rectum, anal canal, pelvic floor muscles, and surrounding organs during rest, squeeze, and defecation.
Dynamic Real-Time Imaging
Captures video-like sequences of pelvic floor movement during straining and evacuation, revealing functional abnormalities static imaging cannot detect.
Pelvic Floor Dysfunction
Diagnoses obstructed defecation, anismus (non-relaxing puborectalis), rectocele, enterocele, and perineal descent syndrome.
Multi-Compartment Assessment
Simultaneously evaluates the anterior compartment (bladder/urethra), middle compartment (vagina/uterus), and posterior compartment (rectum/anal canal).
When Do You Need MRI Defecography?
This advanced study is indicated when conventional tests fail to explain chronic pelvic floor symptoms.

MRI Defecography vs Conventional Defecography
Understanding the advantages of MRI over traditional fluoroscopic defecography helps in selecting the best diagnostic approach.
| Feature | MRI Defecography | Conventional Defecography |
|---|---|---|
| Radiation Exposure | ✓ Zero | ✕ Significant |
| Soft Tissue Detail | ✓ Excellent | ✕ Limited |
| Multi-Compartment View | ✓ All 3 Compartments | ✕ Rectum Only |
| Pelvic Floor Muscles | ✓ Clearly Visualized | ✕ Not Visualized |
| Cost in Bihar | ₹8,000 – ₹15,000 | ₹3,000 – ₹6,000 |
| Patient Comfort | ✓ Supine / Seated Option | ✕ Lateral Decubitus |
MRI Defecography is the modern gold standard for pelvic floor dysfunction — offering radiation-free, multi-compartment evaluation with superior soft tissue detail.
The Procedure
What to expect during your MRI Defecography at JP Diagnostic. Total time: 45 to 60 minutes.
Preparation & Enema
Arrive with an empty bladder. A cleansing enema may be administered to ensure the rectum is empty. You will change into a hospital gown.
Contrast Instillation
A soft catheter is inserted into the rectum to instill ultrasound gel mixed with Gadolinium (approximately 120-180 ml). The gel simulates stool consistency. The catheter is then removed.
Dynamic MRI Scanning
You lie supine in the MRI machine. The technologist instructs you through three phases: (1) Rest — relax completely, (2) Squeeze — tighten pelvic floor as if holding urine, (3) Defecate — bear down and evacuate the gel. Each phase is captured in rapid MRI sequences.
Post-Scan & Results
The table slides out. You can use the restroom to evacuate any remaining gel. Dress and collect belongings. Preliminary review by our radiologist. Final report within 24-48 hours.
How to Prepare
Proper preparation ensures clear images and accurate functional assessment of your pelvic floor.
✅ Do’s
- Take a laxative or enema the night before as advised
- Come with an empty rectum and bladder
- Wear loose, comfortable clothing
- Bring prior colonoscopy, manometry, or surgical reports
- Inform staff of any rectal surgery or hemorrhoids
- Arrive 30 minutes before appointment
- Bring an escort if you feel anxious about the procedure
❌ Don’ts
- Do not eat a heavy meal within 4 hours of the scan
- Avoid scheduling during active rectal bleeding
- Do not take constipating medications on scan day
- Avoid wearing undergarments with metal clasps
- Do not apply rectal creams or suppositories on scan day
- Avoid vigorous exercise before the scan
Special Cases
| Condition | Required Preparation |
|---|---|
| Claustrophobia | Request wide-bore MRI or mild sedation |
| Pregnancy | Generally contraindicated; discuss with referring doctor |
| Active Hemorrhoids | Inform staff; gel instillation is gentle and safe |
| Post-Surgical Rectum | Bring operative notes; modified protocol may be used |
| Elderly / Frail Patients | Escort required; extra time allocated |
Duration & Report Timing
Comprehensive pelvic floor evaluation requires dedicated time for accurate diagnosis.
| Scan Type | Duration |
|---|---|
| MRI Defecography (Standard) | 45 – 60 minutes |
| MRI Defecography + Pelvic Floor | 60 – 75 minutes |
| Dynamic Pelvic MRI (Female) | 50 – 65 minutes |
| Post-Surgical Follow-up | 45 – 55 minutes |
Safety & Risks
MRI Defecography is safe and well-tolerated, with specific considerations for the rectal contrast procedure.
- No radiation exposure
- Safe for repeated follow-up studies
- Ultrasound gel is inert and harmless
- No recovery time needed
- Mild rectal fullness during the scan
- Temporary urge to defecate during instillation
- Loud acoustic noise during MRI
- Claustrophobia in closed-bore magnets
- Active severe rectal bleeding or proctitis
- Recent rectal surgery (within 2 weeks)
- Pacemaker or non-MRI-safe implants
- Severe claustrophobia without sedation option
- Inability to follow breathing instructions
💡 Comfort Note: Our female patients can request a female technologist. The rectal catheter insertion is performed gently with adequate lubrication and privacy. You may pause the scan at any time if uncomfortable.
Frequently Asked Questions
Everything you need to know about MRI Defecography in Bihar.
No. The procedure is not painful. You may feel a mild sensation of fullness when the gel is instilled into the rectum, similar to the urge to pass stool. The MRI scan itself is painless. Most patients tolerate it well.
Colonoscopy evaluates the inner lining of the colon and rectum. MRI Defecography evaluates the function and structure of the pelvic floor during defecation. They are complementary — colonoscopy rules out tumors, while defecography diagnoses mechanical and functional causes of constipation.
It diagnoses rectocele, enterocele, rectal prolapse (intussusception), anismus (paradoxical puborectalis), perineal descent, sigmoidocele, and cystocele. It also grades the severity of these conditions to guide surgical versus conservative management.
Yes. The key to this study is capturing the dynamic process of evacuation. You will be asked to bear down and expel the instilled gel while in the MRI machine. This is essential for detecting functional abnormalities like anismus or rectal prolapse.
None. Unlike conventional fluoroscopic defecography which uses X-rays, MRI Defecography uses magnetic fields and radio waves. There is zero radiation exposure, making it safe for younger patients and repeated follow-up studies.

Expert Opinions
When do leading specialists recommend MRI Defecography?
Patients with chronic constipation who do not respond to fiber and laxatives should undergo MRI Defecography. It is the only test that reliably distinguishes between slow-transit constipation and obstructed defecation — two conditions requiring entirely different treatments.
For rectal prolapse and rectocele, MRI Defecography provides the surgeon with a roadmap. It shows whether the prolapse is mucosal or full-thickness, whether there is associated enterocele, and whether the puborectalis relaxes normally — all critical for surgical planning.
In female patients with combined pelvic floor dysfunction, MRI Defecography evaluates all three compartments simultaneously. This prevents the need for multiple separate tests and gives the urogynecologist a complete picture for comprehensive repair.
